You may use your computer or your phone to securely send us attachments such as photos (JPG, PNG) and documents (PDF).
- For a photo ID, we need only an image of its front.
- For an insurance card, we need two images — its front and back.
Using your Computer:
- Copy or move the files you wish to upload to your Desktop or a folder that you can find easily.
- Login to our Secure Patient Portal. You should see the Home Page. Click on Create Message.
3. A new message window will open. The To: field is already pre-filled.
4. Click add attachment, and find the folder where the files are. You can select more than one file to upload (click Control or Command while clicking on selected files). When the files successfully upload, you’ll see their names.
5. Notice that the Send button is grayed out. The system requires you to write a Subject line and a Message.
6. The Send button is now activated. Click Send. That’s it!
Using your Phone:
- Login to our Secure Patient Portal. You should see the Home Page. Click on the + sign to create a message.
2. A new message window will open. The To: field is already pre-filled.
3. Click on the paper clip to add attachments. You’ll be given a choice of where to find the images, which will look something like this:
4. Use the camera to take photos of the cards you wish to send us, making sure they’re in focus and capture the entire card. Remember to take photos of the front and back of your insurance card. If you’ve already taken photos, locate them by clicking on the Files icon.
5. When the files successfully upload, you’ll see their names. Notice that the Send button is grayed out. The system requires you to write a Subject line and a Message.
6. The Send button is now activated. Click Send. That’s it!
